Job summary

Aus Care Community Services is seeking a part-time, permanent Clinical Engagement and Partnerships Coordinator to drive referrals and partnerships across Sydney. The role is based at the Concord office with travel across Sydney and a 9am–5pm schedule on Monday, Wednesday, and Friday.

You’ll own the front door, building and maintaining relationships that bring participants to us, and ensuring every referral is handled promptly and accurately by the team.

Qualifications

• Experience in referral management or partnerships in health or disability settings.
• Solid understanding of the NDIS and discharge processes.
• Strong stakeholder engagement and follow-up discipline.

Responsibilities

• Build and maintain referral relationships with hospitals, discharge planners, clinical teams and community organisations.
• Manage referrals as they come in — triage, respond, gather required information and hand over to the service team.
• Represent Aus Care Community Services at meetings and with referring professionals.
• Track referral sources and outcomes and report on metrics.
• Collaborate with the Managing Director to prioritise relationships and areas for growth.
• Ensure referral conversations reflect participant choice and conflict of interest obligations.

Clinical Engagement and Partnerships Coordinator

This is a part-time, permanent position, Monday, Wednesday and Friday, 9am to 5pm, based at our Concord office with travel across Sydney. You'll own the front door — building and holding the relationships that bring participants to us, and making sure every referral that arrives is handled properly and quickly.

Key responsibilities

• Build and maintain referral relationships with hospitals, discharge planners, clinical teams, allied health and community organisations
• Manage referrals as they come in — triage, respond, gather what's needed and hand over cleanly to the service team
• Represent Aus Care Community Services at meetings, network events and with referring professionals
• Track where referrals come from and what happens to them, and report on it
• Work with the Managing Director on which relationships and areas to prioritise
• Make sure referral conversations reflect participant choice and our conflict of interest obligations

About you

• A background in disability, health, mental health or community services, with credibility in front of clinical professionals
• Experience in referral management, intake, partnerships or business development in a health or disability setting
• Solid working knowledge of the NDIS, particularly how participants come into the scheme and what happens at discharge
• Confidence starting conversations with people you don't know yet
• Organised follow-up — the relationships are built in the second and third contact, not the first
• Current NDIS Worker Screening Check, Working with Children Check, driver's licence and comprehensively insured vehicle
