Job summary

Ability Street Community Services – Blacktown NSW is seeking an enthusiastic Care Coordinator with NDIS sector experience to lead our team of Support Workers and SIL homes across Sydney. You will ensure efficient client management and delivery of person-centred supports in community settings.

Key duties include rostering staff to match client needs, completing paperwork, liaising with Participants, families and referrers, and reporting incidents per NDIS guidelines.

Qualifications

• At least 12 months of experience in a disability or community services role.
• Certificate III or higher in Disability or Community Services preferred.
• Strong customer service and written/verbal communication skills.
• Experience with rostering systems and familiarity with Microsoft 365.
• Willingness to travel to SIL houses and various Sydney locations.

Responsibilities

• Lead and manage Support Workers, Participants, and SIL homes.
• Roster direct support staff to align with client needs and SIL homes.
• Complete paperwork including medication orders, transport, and logs.
• Match skilled workers to clients per care plans and monitor service delivery.
• Supervise staff and foster relationships with Participants and referrers.
• Support Participants and families to understand services and policies.
• Build connections within the disability and community care sector.
• Respond to emails professionally with stakeholders.
• Plan and prepare for meetings with measurable outcomes.
• Report incidents in line with NDIS guidelines.
