SILC (Supporting Independent Living Co-operative) – Sydney CBD NSW is seeking a compassionate Support Coordinator to help NDIS participants implement plans and build independence. You will work with participants, families, and service providers to coordinate supports and manage budgets to ensure sustainable outcomes.

Requirements include at least 1 year in the disability sector and a Diploma in a related human services field, with knowledge of NDIS funding and person-centered planning.

Qualifications

• Minimum 1 year experience in the disability sector.
• Diploma in a related human services field.
• Knowledge of NDIS funding categories.
• Understanding of person-centered planning approaches.
• Experience or interest in working with people with disability.
• Good organisational skills with ability to prioritise tasks.
• Skills in budget management and record-keeping.
• Developing understanding of disability services and quality standards.
• Ability to build rapport with participants, families and service providers.
• Problem-solving skills and a solutions-focused approach.
• Willingness to learn and adapt in a changing environment.

Responsabilités

• Understand participants' NDIS plan and funding categories.
• Identify needs, challenges, and goals.
• Connect to services and supports that are needed.
• Build skills to coordinate supports and services.
• Coordinate stakeholders to ensure cohesive support delivery.
• Assist with plan reviews and gathering evidence of outcomes.
• Liaise with the NDIA and other government agencies as needed.
• Monitor the quality and effectiveness of supports.
• Support participants to build capacity and increase independence.
• Identify opportunities for community engagement and social participation.
• Navigate significant life transitions.
