St James Support Services in Melbourne is seeking a Clinical Care Coordinator (RN) to oversee clinical care and participant wellbeing across our disability facilities and SIL homes. The role requires nursing leadership, staff supervision, and strong bilingual communication.

You will manage care plans, liaise with healthcare professionals, ensure compliance with NDIS Practice Standards, and drive continuous quality improvement.

Qualifications

• Current AHPRA registration as a Registered Nurse (RN).
• Clinical nursing experience with management or supervisory responsibilities.
• Experience in disability services, SIL or aged care settings.
• Fluent in English and Chinese (written and spoken).
• Strong leadership, communication and organisational skills.
• Current NDIS Worker Screening Clearance or ability to obtain one.

Responsabilités

• Oversee and coordinate clinical care and support services across residential disability facilities and SIL houses.
• Assess participant care needs and ensure appropriate care and support are provided.
• Monitor clinical care quality and ensure individual support requirements are met.
• Prepare, review and maintain NDIS care and funding documentation and reports.
• Coordinate healthcare services and liaise with medical and allied health professionals.
