Job summary

Golden Heart Services in Greater Sydney is seeking a Client Services Coordinator to lead the day-to-day delivery of high-quality supports in SIL homes and community services. You will supervise Support Workers, coordinate care and rosters, and ensure compliance with NDIS Practice Standards.

You'll work closely with clients, families and providers, stepping in on shift when needed to maintain continuity of care and service quality across a 24/7 operation.

Qualifications

• Experience coordinating or leading a care team in disability services.
• Strong understanding of NDIS Practice Standards and safeguarding.
• Proven ability to roster, supervise staff and manage complex schedules.
• Client-centred approach with problem-solving and conflict resolution.
• Proficiency with rostering software and client management systems.

Responsibilities

• Lead, supervise and support a team of Support Workers, including rostering and performance management.
• Assess client needs and develop individualised support plans for independence.
• Coordinate services including personal care, meals, medication and household tasks.
• Connect clients with education, health and community resources.
• Be the main contact between clients, families, coordinators and providers.
• Ensure compliance with NDIS standards, WHS, emergency response and infection control.
• Maintain accurate client care records and staff performance data.
• Fill in on support shifts to maintain continuity of care.
