Job summary

Qualifications

• AQF Diploma or higher in Community Services or related field.
• At least 12 months relevant work experience as a Community Worker or similar.
• NDIS documentation experience and processes knowledge.
• Experience with vulnerable individuals or culturally diverse communities.
• Current First Aid and CPR certificates.
• Working with Children Check, AFP Police Check and NDIS Worker Screening Clearance.
• Strong communication, advocacy and problem-solving skills.
• Knowledge of person-centred practice, safeguarding and WHS.
• Ability to work independently and collaboratively.

Responsibilities

• Assess participants' needs, goals and coordinate suitable services.
• Coordinate person-centred support with participants, families and providers.
• Provide information about community programs and assist access to supports.
• Liaise with community organisations, health professionals and service providers.
• Support independence and social participation in the local community.
• Monitor progress and adjust support plans as needed.
• Advocate for participants to access services and opportunities.
• Identify barriers to participation and develop practical strategies.
• Build relationships with organisations to improve access to supports.
• Maintain accurate case notes, records and referral information.
• Prepare reports on participant progress and service delivery.
• Work collaboratively with participants, families and stakeholders while maintaining confidentiality.
