Job summary

Smart Disability Care is seeking a Community Worker to assess client needs, develop and implement support programs, monitor progress and liaise with families and service providers. This full-time role is based in NSW and requires a Diploma in Community Services or related field.

You will interview clients, plan education and support programs, coordinate referrals and maintain records with a person-centred approach to promote independence and community participation.

Qualifications

• Diploma or higher qualification in Community Services, Social Work, Disability Services, Welfare or a related discipline.
• At least 2 years relevant professional experience in community services, disability support or a related field.
• Experience in client assessment, support planning and service coordination.
• Knowledge of disability, health, welfare and community support services.
• Strong communication, documentation and stakeholder liaison skills.

Responsibilities

• Assess clients' individual needs, circumstances, goals and support requirements and contribute to the development of appropriate support plans.
• Interview clients and their families or carers to assess needs and degree of difficulties affecting wellbeing and independence.
• Plan, develop and implement educational, training and support programs tailored to clients' needs and goals.
• Provide information and practical support to help clients and families understand available services.
• Monitor progress and maintain accurate client records and prepare reports.
