Caring Arms Australia is seeking a proactive Support Coordinator to help NDIS participants in the Central Coast and Lake Macquarie regions. You will develop supports, coordinate services across providers, and monitor progress toward goals in a person-centred approach.

  • Valid driver’s license and willingness to travel regionally.

Company Description Caring Arms Australia is a registered NDIS provider serving the Central Coast and Lake Macquarie regions of NSW. As a locally based organisation with a local management team, the focus is on understanding the specific needs of the community. The team is committed to delivering personalised, person-centred support to NDIS participants and their families. Caring Arms Australia aims to build long-term, trusting relationships while helping individuals achieve greater independence and quality of life.

Role Description The Support Coordinator is a full-time, hybrid role based in Central Coast, NSW, with some flexibility to work from home. Day-to-day, the Support Coordinator will assist NDIS participants to understand their plans, identify appropriate services, and coordinate supports across multiple providers. Responsibilities include developing support plans, managing service bookings, monitoring progress toward goals, and resolving issues or barriers to service delivery. The role involves regular communication with participants, families, and external stakeholders, maintaining accurate records, and ensuring services align with NDIS guidelines and organisational policies. The Support Coordinator will also provide guidance on available resources, advocate for participant needs, and contribute to continuous improvement of support coordination processes.

Qualifications

  • Strong interpersonal skills and the ability to build respectful, collaborative relationships with participants, families, and providers.
  • Excellent communication skills, including clear written documentation and confident verbal communication with diverse stakeholders.
  • Experience with case management, including planning, coordinating services, and monitoring outcomes for individuals with complex needs.
  • Customer service capabilities, with a focus on responsiveness, empathy, and problem-solving in participant-facing situations.
  • Basic technical support skills and confidence using digital tools, client management systems, and online NDIS platforms.
  • Knowledge of the NDIS framework, disability sector, and person-centred practice.
  • Ability to work independently, manage competing priorities, and maintain accurate records in a hybrid work environment.
  • Relevant qualification in community services, social work, disability, or a related field, or equivalent experience.
  • Valid driver’s licence and willingness to travel within the Central Coast and Lake Macquarie regions as required.
