Role Description The Support Coordinator is a full-time, hybrid role based in Central Coast, NSW, with some flexibility to work from home. Day-to-day, the Support Coordinator will assist NDIS participants to understand their plans, identify appropriate services, and coordinate supports across multiple providers. Responsibilities include developing support plans, managing service bookings, monitoring progress toward goals, and resolving issues or barriers to service delivery. The role involves regular communication with participants, families, and external stakeholders, maintaining accurate records, and ensuring services align with NDIS guidelines and organisational policies.
