Job summary

P2P Care Services is seeking a dedicated Community Worker to assess participant needs, coordinate services and liaise with families and providers in Campbelltown, NSW. The role focuses on enabling independence, social participation and access to community resources.

You will work collaboratively with participants, carers and external stakeholders, maintaining confidentiality and professional boundaries while supporting person-centred outcomes and quality service delivery.

Qualifications

  • AQF Diploma or higher in Community Services, Community Development, Welfare, Social Services or a related field.
  • At least 12 months relevant work experience as a Community Worker or in a similar community services role.
  • At least 12 months relevant work experience in case management, report preparation, and submission processes including NDIS documentation.
  • Experience working with vulnerable individuals, people with disabilities, or culturally diverse communities.
  • Experience dealing with mental and emotional disturbances and developmental disability.
  • Experience with government agencies, non-profits and other service providers.

Responsibilities

  • Assess participants' needs, circumstances and goals and identify appropriate community, welfare, disability and social support services.
  • Develop and coordinate person-centred support arrangements with participants, families, carers and providers.
  • Provide information about available community programs and help access supports.
  • Liaise with community organisations, government agencies, health professionals and service providers to coordinate services.
  • Support participants to increase independence, social participation and engagement with their local community.
  • Monitor progress and review support arrangements as needs change.
  • Advocate for participants to access suitable services and resources.
  • Identify barriers to participation and help participants overcome them.
  • Build and maintain professional relationships with community organisations to improve access to supports.
  • Maintain accurate case notes, records and referrals; prepare progress reports.
  • Collaborate with participants, families and stakeholders while maintaining confidentiality and boundaries.
