Job summary

People Who Care in Guildford seeks an experienced Care Coordinator – Support at Home to oversee wellness-focused services under the Australian Government’s Support at Home program. This full-time fixed-term role is based in Guildford and requires strong coordination, communication, and aged care experience.

The role focuses on client-centred planning, service delivery, and collaboration with clients, families, and care providers to promote independence and safety at home.

Qualifications

• Certificate IV or higher qualification in Aged Care, Disability, Individual Support, Community Services or a related discipline.
• Experience in aged care, home care or community services at a coordination level.
• Understanding of the Support at Home program and aged care funding.

Responsibilities

• Coordinate and administer Support at Home services in line with program requirements and Aged Care Quality Standards.
• Plan, coordinate, budget manage and administer client funding.
• Develop and review individualised service delivery plans with clients and networks.
• Coordinate services with clients, support workers, health professionals and families.
• Maintain accurate client records and program data.
• Monitor service delivery, expenditure and budgets to ensure quality and funding compliance.
