Southern Cross Care WA is seeking a dedicated home care coordinator to manage a client portfolio and coordinate tailored services that improve wellbeing. You will act as the key link between clients, families and care teams to ensure seamless, person-centred care.

The role is predominantly work-from-home with travel to clients north of the river, requiring strong organisation, communication and compliance with funding frameworks to achieve great outcomes.

Qualifications

• Experience in aged care or home care coordination.
• Strong organisational skills with the ability to manage multiple priorities.
• Excellent communication and relationship-building skills.
• Knowledge of case management, compliance, and funding frameworks.
• A compassionate, person-centred approach.
• Confidence using systems such as Microsoft Office.
• Certificate IV in Ageing Support or equivalent qualification.

Responsabilités

• Coordinating personalised home care services that support independence and wellbeing.
• Building strong, trusting relationships with clients, families, and stakeholders.
• Monitoring and adjusting care plans to meet changing needs.
• Managing client budgets and ensuring funding is used effectively.
• Collaborating with internal teams and external providers for integrated care.
• Ensuring compliance, quality standards, and accurate documentation.
• Contributing to continuous improvement and client satisfaction.
