Job summary

MedHealth in Gunnedah, NSW, is seeking a Behaviour Support Practitioner to support NDIS participants and families. You will own assessment, develop and deliver evidence-based support plans within a multidisciplinary team, driving outcomes and reducing restrictive practices.

Qualifications

• Degree qualification as an allied health professional.
• Experience in report writing.

Responsibilities

• Supporting NDIS participants and families to identify their needs via assessment and person-centred planning.
• Owning assessment, development and delivery of evidence-based support plans.
• Driving goals and measurable outcomes using a positive behaviour framework and reducing restrictive practices.
• Ensuring plans reflect participant goals, strengths and priorities.
• Building capability and confidence of those supporting participants day-to-day.
• Collaborating within a cohesive multidisciplinary team.
