Qualifications

• Degree in an allied health field and eligible for practice.
• Experience in report writing and case documentation.

Responsibilities

• Support NDIS participants and families to identify needs via assessment and planning.
• Lead assessment, development and delivery of evidence-based support plans.
• Drive meaningful goals and outcomes with a positive behaviour framework.
• Ensure plans reflect participant goals and what matters most.
• Collaborate within a multidisciplinary team and build capacity of staff.
