Job summary

Ability Action Australia is seeking a Behaviour Support Practitioner to work with NDIS participants, families and carers, delivering person-centred assessments and plans.

You will lead assessments, develop evidence-based support plans, drive meaningful goals, and work within a multidisciplinary team while mentoring less experienced practitioners.

Qualifications

• Degree qualification as an allied health professional.
• Experience in report writing.
• Strong organisational skills and the ability to manage a varied caseload.
• A proactive, self-motivated approach, with a drive to improve outcomes and test new ideas.
• A genuine passion for working in the disability sector.

Responsibilities

• Supporting NDIS participants and families to identify needs through assessment and person-centred planning.
• Owning the assessment, development and delivery of evidence-based support plans within a complex support environment.
• Driving meaningful goals and measurable outcomes using a positive behaviour support framework.
• Ensuring plans reflect each participant’s goals, strengths and what matters most to them.
• Building the capability, confidence and understanding of those supporting participants in their day-to-day lives.
• Working collaboratively within a multidisciplinary team, including mentoring less experienced practitioners.
