Job summary

MedHealth in Gunnedah, NSW, is seeking a Behaviour Support Practitioner to support NDIS participants and families. You will own assessment, develop and deliver evidence-based support plans within a multidisciplinary team, driving outcomes and reducing restrictive practices.

Responsibilities

• Supporting NDIS participants and families to identify their needs via assessment and person-centred planning.

• Ensuring plans reflect participant goals, strengths and priorities.
• Building capability and confidence of those supporting participants day-to-day.
• Collaborating within a cohesive multidisciplinary team.

In this role you will:

• Supporting NDIS participants, along with their families and carers, to identify their needs through assessment and person-centred planning
• Owning the assessment, development and delivery of evidence-based support plans within a complex support environment
• Driving meaningful goals and measurable outcomes, using a positive behaviour support framework and a commitment to eliminating restrictive practices
• Ensuring plans reflect each participant's goals, strengths and what matters most to them
• Building the capability, confidence and understanding of those supporting participants in their day-to-day lives
• Working collaboratively within a cohesivemultidisciplinary team environment

Qualifications

To be successful in this role you will have:
