Job summary

MedHealth Group is seeking a Behaviour Support Practitioner to support NDIS participants across the Inner South Canberra area. The role requires current suitability at Core, Proficient, or Advanced level and offers ownership of assessment and delivery of evidence-based plans within a multidisciplinary team.

You will work to identify participant needs, drive goals, and reduce restrictive practices, while building capability among support staff.

Qualifications

• Evidence of suitability at Core/Proficient/Advanced level with the NDIS Quality & Safeguarding Commission.
• Experience in report writing, strong organisational skills and the ability to manage a varied caseload.
• A proactive, self-motivated approach, with a drive to improve outcomes and test new ideas.
• A genuine passion for working in the disability sector.

Responsibilities

• Supporting NDIS participants, along with their families and carers, to identify their needs through assessment and person-centred planning.
• Owning the assessment, development and delivery of evidence-based support plans within a complex support environment.
• Driving meaningful goals and measurable outcomes, using a positive behaviour support framework and a commitment to eliminating restrictive practices.
• Ensuring plans reflect each participant’s goals, strengths and what matters most to them.
• Building the capability, confidence and understanding of those supporting participants in their day-to-day lives.
• Working collaboratively within a cohesive multidisciplinary team environment.
