Job summary

InLife Independent Living Ltd in Geelong is seeking a Coordinator to lead a team of disability support workers, manage client relationships and coordinate care rosters across client homes and SIL settings. You will mentor staff and partner with Client Service Managers to deliver flexible, person-centred support.

This role combines on-shift duties with roster coordination, requires a driver’s licence for territory travel, and offers a generous salary package with salary packaging.

Qualifications

• A successful track record in disability/health/community services roles with direct client support and team coordination.
• Experience rostering and using record management systems.
• Strong client-focused care and collaborative approach.
• Active listening and relationship-building with clients and teams.
• Tech-savvy with cloud-based rostering and incident reporting tools.
• Excellent written and verbal communication with staff, clients and families.

Responsibilities

• Lead a team of InLife Assistants to deliver great service.
• Coach and mentor the team of InLife Assistants.
• Develop and maintain positive relationships with clients and their networks.
• Work with clients to support goals and positive outcomes.
• Collaborate with Client Service Managers to manage rosters and induction for teams.
• Provide ongoing staff training and client-specific coaching.
• Maintain client records and monitor funded support budgets with finance team.
• Contribute to ongoing improvement of policies and procedures.
• Participate in the on-call after-hours coordinator roster.
• Work shifts with clients.
