Job summary

Help In Support in Melton, VIC is seeking a compassionate Team Leader for Supported Independent Living (SIL). This part-time role leads a team delivering person-centred disability support to clients living independently with assistance within the community.

You will coordinate rostering, develop individualised plans, ensure regulatory compliance and support staff wellbeing while promoting autonomy and dignity for clients.

Qualifications

Experience in disability support or aged care with supervisory responsibilities.
Knowledge of NDIS frameworks and person-centred care.
Strong communication and interpersonal skills.

Responsibilities

Supervising and mentoring support workers to ensure quality care.
Developing and implementing individualised support plans.
Managing rostering, scheduling and workload allocation.
Conducting regular check-ins with clients to assess satisfaction and changing needs.
Providing direct support when required, including personal care and life skills development.
Maintaining accurate client records in line with regulatory requirements.
Liaising with families and health professionals to coordinate care.
Identifying performance issues and providing coaching.
