Job summary

Centacare North Queensland is seeking a dedicated professional to deliver outreach and case management for participants who are homeless or at risk. You will assess needs, coordinate referrals, and advocate for access to secure housing options while maintaining data records and complying with policy requirements.

You will join a values-driven team, engage with diverse communities, and contribute to program development and education.

Qualifications

• Degree-level qualification in Social Work, Human Services or Behavioural Sciences or Certificate level 4+ in the community sector with at least two years' case management experience.
• Excellent written and verbal communication skills, plus strong analytical abilities.
• A current Driver's Licence is required and eligibility for Disability Worker Screening and Working with Children Blue Card.
• Experience in culturally appropriate community work, assessment and referral is desirable.

Responsibilities

• Provide case management support and develop early intervention strategies to engage participants.
• Undertake intake, triage and initial assessment to determine risk, urgency and program eligibility.
• Utilise electronic data systems to record and maintain participant information according to policies.
• Develop and maintain community partnerships through referrals and stakeholder promotion.
• Attend meetings, supervision and professional development activities.
• Undertake outreach and community engagement activities as required.
• Contribute to program development and community education.
• Comply with WH&S, privacy, EEO, and Quality Management System requirements.
