Job summary

Wesley Mission Queensland Limited is seeking a Case Manager for the Lockyer Valley Housing Service to support individuals and families at risk of homelessness. You will assess needs, provide holistic case management, advocacy and referrals to secure and sustain housing, while coordinating with community supports.

The role involves centre-based, outreach and remote service delivery, with a focus on client-centred care and collaboration within a multidisciplinary team.

Qualifications

• Relevant qualification in Human Services, Social Work, Community Services or related field, or equivalent sector experience.
• Experience in assessment, case management, advocacy and referral pathways within homelessness, housing or community services.
• Strong communication and relationship-building skills.
• Ability to manage a varied caseload with autonomy.
• Proficient in Microsoft Office and client management systems, with strong administration, record-keeping and reporting skills.

Responsibilities

• Assess eligibility, needs and risks of individuals and families experiencing or at risk of homelessness.
• Provide holistic case management, including support planning, advocacy and tenancy sustainment assistance.
• Coordinate services and referrals to ensure clients can access support.
• Build and maintain relationships with community partners and stakeholders.
• Deliver a high-quality, client-centred service and maintain accurate records in line with policy.
• Maintain client records and program data in line with organisational requirements.
• Collaborate within a multidisciplinary team and participate in meetings and development.
• Support a safe, inclusive, values-driven workplace and mentor volunteers or placements.
