Job summary

Mission Australia is seeking a Case Manager for Cairns to join the Street to Home Immediate Housing Response team. Based in Bungalow, Cairns, you will help people experiencing homelessness access safe, stable housing and supportive services.

You will provide person-centred case management for clients with complex needs, build relationships with partners, and maintain accurate records to support service delivery and reporting.

Qualifications

• Experience providing case management to people with complex needs, ideally homelessness or housing.
• Ability to manage a caseload and deliver outcomes in a fast-paced environment.
• Person-centred, strengths-based and trauma-informed practice understanding.
• Strong communication, relationship-building and advocacy skills.
• Maintain professional boundaries and collaborate with stakeholders.
• Current unrestricted Driver's licence and First Aid/CPR certification.
• National police history check and working with children/vulnerable people clearance.

Responsibilities

• Support people experiencing homelessness to achieve housing goals.
• Provide person-centred case management for clients with complex needs.
• Connect clients with housing, health, financial, community and support services.
• Advocate for clients and help navigate systems and barriers.
• Build relationships with housing providers, community organisations, and stakeholders.
• Maintain accurate client records and contribute to reporting and improvement.
