Job summary

Woden Community Service (WCS) is seeking a Housing and Community Connections Case Worker to support individuals and families facing homelessness and housing instability across ACT. You will coordinate services, advocate for clients and help them access housing, health and community supports.

The role involves delivering tenancy assistance programs, community workshops and collaborative work with housing providers and government agencies, within a multidisciplinary team to improve housing

Qualifications

• Qualifications in Social Work, Community Services, Human Services or related field.
• Experience with homelessness, housing insecurity or complex needs.
• Knowledge of tenancy support and housing pathways.
• Strong communication and advocacy skills.
• Experience facilitating groups or community programs.
• Excellent organisational and case management skills.
• Ability to work independently and in multidisciplinary teams.

Responsibilities

• Provide person-centred, trauma-informed case management and tenancy support.
• Conduct assessments and develop tailored support plans.
• Assist participants to access housing, health and community services.
• Advocate for individuals and coordinate services to achieve outcomes.
• Support tenancy sustainment and housing stability.
• Facilitate community engagement activities, workshops and group programs.
• Build partnerships with housing providers, government agencies and community organisations.
• Maintain accurate case notes, records and reporting requirements.
