Woden Community Service Inc in Canberra is seeking a Senior Therapeutic Case Manager for Housing and Community Connections. You will deliver intensive therapeutic case management and coordinated support to people facing homelessness, housing instability, and mental health challenges, working across TAP and Common Ground to achieve sustainable housing outcomes.

You will lead practice, mentor colleagues, build partnerships with government and community agencies, and develop person-centred support

Formación

• Qualification in Social Work, Community Services, Psychology, Human Services, or related discipline (Diploma level or above).
• Experience providing therapeutic case management to people experiencing homelessness, housing insecurity, and mental health challenges.
• Strong understanding of recovery‑oriented, trauma‑informed, and strengths‑based practice frameworks.
• Ability to undertake comprehensive assessments, develop support plans, and coordinate multidisciplinary responses.
• Knowledge of housing and homelessness service systems; ability to coordinate care planning.

Responsabilidades

• Deliver intensive therapeutic case management and recovery‑oriented support to participants.
• Develop person‑centred support plans and coordinate complex care responses.
• Collaborate with housing providers, government agencies, health services and community organisations.
• Provide practice leadership, mentoring, and specialist guidance to colleagues.
• Maintain thorough case documentation and advocacy to achieve stable housing outcomes.
