Woden Community Service (WCS) is seeking a passionate Family Case Manager to join our Children, Youth and Family Services team in the Australian Capital Territory. You will provide flexible, person-centred case management and early intervention support to vulnerable children, young people and families facing complex challenges.

The role emphasizes strong collaboration with government and community service providers, maintaining case notes, assessments and referrals, and contributing to program

Formación

• Experience supporting children, young people and families through case work, outreach and case management.
• Strong understanding of person-centred practice and strengths-based approaches.
• Excellent communication, relationship-building and stakeholder engagement skills.
• Experience completing assessments, referrals, case notes and client documentation.
• Understanding of professional boundaries, privacy and confidentiality.
• Qualifications in Social Work, Community Services or a related field, or equivalent experience (desirable).

Responsabilidades

• Provide case management, advocacy and practical support to children, young people and families.
• Build strong relationships and support service users to achieve their goals.
• Conduct assessments, referrals and service coordination.
