Job summary

The Salvation Army Australia is seeking a Housing Case Manager in Horsham, VIC. This permanent part-time role is 28.5 hours per week and on-site, reporting to the Team Leader. The position involves assessing, supporting, and case managing clients who are homeless or at risk, with potential back-up support across programs and travel to surrounding areas.

Compensation is SCHADS Award Level 5. The role offers hourly remuneration of $53.43–$55.85 plus superannuation, with several benefits available

Qualifications

• A tertiary qualification in Social Work or related field and/or substantial experience.
• Experience in working with vulnerable people is desirable.
• A current Victorian Drivers Licence is required.
• A current and valid Employee Working with Children’s Check is required.

Responsibilities

• Conduct comprehensive client intake assessments and connect clients to information and referrals.
• Provide outreach support and attend alternative service locations as needed.
• Deliver practical and emotional support through ongoing client engagement and case plans.
• Assist clients to make housing decisions by developing exit plans and gathering feedback.
• Coordinate referrals to community services, day programs, and employment pathways.
• Maintain accurate case notes, reports and data entry; participate in CQI activities.

Education

Tertiary qualification in Social Work or related field

Current Victorian Driver's Licence
