Zusammenfassung

The Salvation Army in Horsham, VIC is seeking a Housing Case Manager to support people experiencing homelessness or at risk of homelessness. This permanent part-time role is 0.75 FTE (28.5 hours per week) and is based on-site in Horsham.

You will assess needs, provide outreach, develop holistic case plans, and coordinate referrals to community services to promote independent housing outcomes. Applicants should hold a tertiary qualification in Social Work or a related field, have experience with

Qualifikationen

• A tertiary qualification in Social Work or a related field.
• Experience in working with vulnerable people.
• A current Victorian Drivers Licence.
• A current and valid Employee Working with Children’s Check.

Aufgaben

• Conduct comprehensive client intake assessments, ensuring individuals are connected to appropriate information, support services and referral pathways
• Provide outreach support and attend alternative service locations as required to meet client needs and enhance engagement
• Deliver practical and emotional support through ongoing client engagement, developing holistic, strengths-based case plans that empower clients to achieve their goals
• Assist clients to make informed and sustainable housing decisions by developing realistic housing and exit plans, conducting exit interviews, and gathering client feedback
• Coordinate referrals to community services, day programs, training opportunities, vocational pathways, job networks and employment programs to promote independence and wellbeing
• Maintain accurate case notes, reports and data entry, participate in team meetings and continuous quality improvement (CQI) activities, and adhere to organisational policies, procedures and OHS requirement
