The Salvation Army is seeking a Case Manager to support individuals and families experiencing homelessness. You will deliver person-centred case management, conduct thorough assessments, manage referrals and outreach, and connect clients with housing and essential community services.

This is a permanent part-time role based on-site in Shepparton, VIC. You will bring a tertiary qualification in Social Work or related field, plus experience in homelessness or housing services, strong stakeholder

Qualifikationen

• Tertiary qualification in Social Work or related field or equivalent experience.
• Experience working in homelessness, housing or community services.
• Ability to develop tailored support plans addressing client needs.
• Experience applying risk assessment frameworks.
• Strong stakeholder engagement and collaboration skills.
• Current Employee Working with Children’s Check.
• Current Victorian Driver’s Licence.

Aufgaben

• Deliver strengths-based, person-centred case management support to individuals and families experiencing or at risk of homelessness.
• Conduct comprehensive client assessments, manage referrals and proactive outreach.
• Develop and review case management plans; connect clients with housing and services.
• Advocate for clients and collaborate with external service providers to ensure coordinated support.
• Maintain accurate client records and meet compliance and funding requirements; contribute to team activities.
