Job summary

3D Recruit is seeking an experienced Home Care Service Manager to oversee day-to-day service delivery, support and develop a team, and help grow the business in Perth.

The role combines hands-on leadership with operations, client coordination and partnership development with families and referral partners.

You will ensure quality, compliance and continuous improvement in an aged care home care setting, delivering person-centred services to older Australians at home.

Qualifications

• Experience in home care management.
• Strong care coordination capabilities.
• Proven ability to coach and develop staff.
• Excellent communication with clients, families and partners.
• Solid understanding of aged care quality requirements, compliance and risk management.

Responsibilities

• Leading, coaching and supporting a team to deliver high-quality home care services.
• Overseeing client services, care coordination and day-to-day operational activities.
• Supporting effective care planning, service reviews and appropriate utilisation of client funding.
• Monitoring service delivery, client satisfaction and team performance.
• Managing complex client matters and supporting positive outcomes for clients and families.
• Identifying opportunities to improve processes, efficiencies and service quality.
• Building relationships with referral partners and community stakeholders to support business growth.
• Contributing to compliance, risk management and continuous improvement.
