Job summary

Premier Care of NSW PTY LTD is seeking a compassionate Family Support Worker to assist welfare workers, social workers and coordinators by providing personalised care and support to clients and families. Qualifications

• Diploma or higher qualification in Health, Community Services, Disability, or related field.
• Minimum one (1) year of full-time relevant experience.
• Strong verbal and written communication skills.
• Proven ability to work effectively within a multidisciplinary and culturally diverse team.
• Understanding of NDIS and Aged Care frameworks, and client-centered care principles.

Responsibilities

• Interview participants and assess the nature and extent of their difficulties.
• Gather information from NDIS and My Aged Care Plans to assist Welfare Workers and understand participant circumstances, entitlements and stakeholders.
• Develop and implement tailored support programs that align with existing NDIS/Aged Care plans.
• Monitor clients' progress and provide regular reports to Welfare Workers, Coordinators, and family members with up-to-date records.
• Gather client feedback to enhance service delivery and respond to evolving client needs and preferences.
• Liaise with welfare workers, social workers, case workers and coordinators to ensure coordinated service delivery and refer participants to relevant agencies when required.
