Job summary

Social Futures Ltd in New South Wales is seeking a part-time Program Worker for the Family Connect and Support team on a fixed-term basis. You will work closely with families, children and young people using a person-centred, evidence-based approach to promote self-determination.

Qualifications

• Experience in family support or community services desirable.
• Ability to work collaboratively with service providers and stakeholders.
• Knowledge of culturally safe practice and trauma-informed care.
• Commitment to data accuracy and reporting requirements.

Responsibilities

• Case coordination and supporting families through strength-based capacity building.
• Conducting strength-based assessments considering the family system, cultural context and risks.
• Developing clear, achievable plans to connect families to supports and empower them.
• Providing flexible outreach service delivery across the Northern Rivers, including home visits.
• Providing warm handovers to support meaningful service engagement.
• Maintaining timely case notes and client records aligned with requirements.
• Supporting culturally responsive practice for Aboriginal families and diverse communities.
• Engaging families with warmth, empathy and professional boundaries.
