Aufgaben

• Assess clients’ needs and help develop and provide suitable support, education and training programs.
• Provide practical support and care to families, children and people with disability.
• Monitor clients’ progress and provide ongoing support based on their individual needs.
• Keep records and prepare reports about clients’ progress and support needs.
• Refer clients and families to other health, disability, welfare and community services when they need additional support.
• Work with families, community organisations, government agencies and other service providers to coordinate support for clients.
• Help families understand and access available community and disability support services.
