Job summary

The Australian not-for-profit organisation ECH is seeking a Care Partner Support Officer to work with older people, families and carers to coordinate services that support independence and wellbeing. You will manage a caseload, develop tailored care plans and guide clients through aged care services and funding options.

Qualifications

• Tertiary qualifications in Human or Behavioural Sciences, Social Work, Allied Health, Aged Care, Community Services or related field.
• Experience in client case management
• Current National Police Check (or willingness to obtain)
• Current driver's licence

Responsibilities

• Manage a caseload and act as a trusted contact for clients and advocates
• Undertake wellbeing assessments and develop goal-focused care plans
• Coordinate services and respond proactively as client needs change
• Support clients to navigate aged care services, funding and support options
• Collaborate with clinical and multidisciplinary teams to achieve positive outcomes
• Maintain accurate records and contribute to quality service delivery and continuous improvement
