Job summary

Wyanga Aboriginal Aged Care is seeking a Care Partner in Aged Care Case Management to build trusted relationships with Elders, manage caseloads and coordinate supports that enable ageing at home with dignity. The role emphasises culturally safe practice, collaboration with families, and coordination of services within the Aboriginal community context.

You will assess needs, develop care plans, monitor outcomes and ensure services align with Elders’ goals and funding.

Qualifications

• Certificate IV or higher in Aged Care, Community Services or related field.
• Current National Police Check.
• Current First Aid and CPR certification, or willingness to obtain.
• Knowledge of community aged care and CHSP/CHSPP desirable.
• Experience in Aboriginal Community Controlled Organisation and culturally safe practice desired.

Responsibilities

• Undertake assessments and home visits to understand Elder needs and goals.
• Develop, implement and review care plans with Elders and supporters.
• Coordinate services and referrals and monitor support effectiveness.
• Monitor wellbeing changes, identify risks and escalate as needed.
• Explain services, funding options and enable access to funding.
• Track budgets and collaborate with team to address gaps or changing needs.
• Maintain accurate client records and care documentation.
• Deliver culturally safe, person-centred care in collaboration with families and providers.
