Prompt Care in Melbourne is seeking a compassionate Care Partner to support older Australians to remain independent at home through the Support at Home program. You will be the main point of contact for clients and help identify care needs, coordinating services, and guiding families.

Formación

• National Criminal History Check / Police Check showing no disclosable Court Outcomes or Convictions, must be less than 3 years old

Responsabilidades

• Be the main point of contact for clients receiving the Support at Home program; identify care needs and facilitate services to help older people stay at home.
• Care planning, advocacy for clients on reassessment, and helping clients understand and manage funding.
