Formación

• Experience in aged care case management or care coordination.
• Excellent communication and relationship-building skills.
• Strong attention to detail.
• Confidence managing budgets and balancing participant goals, risk and sustainability.
• A collaborative approach and commitment to delivering high-quality, person-centred care.
