Job summary

Lives Lived Well is seeking a qualified Clinician to join the Tokoko Integrated Recovery Pathways Program. In this role you will deliver flexible, recovery-focused support via ConnectWell to strengthen continuity of care for people accessing alcohol and other drug treatment.

The role may be undertaken remotely, in a hybrid arrangement or from a Lives Lived Well office, with opportunities for professional development and to contribute to innovative digital recovery services that improve

Qualifications

• Qualifications in Social Work, Psychology, Counselling, Occupational Therapy, Nursing, Human Services or related discipline.
• Experience in Alcohol and Other Drug (AOD), Mental Health or community services.
• Ability to engage effectively with people experiencing complex needs.
• Strong communication and digital engagement skills.

Responsibilities

• Deliver recovery-focused support through ConnectWell's digital platform.
• Conduct regular recovery check-ins with individuals preparing for, participating in or transitioning from residential rehabilitation.
• Support ongoing client engagement and continuity of care throughout treatment and recovery.
• Provide follow-up support for individuals following discharge from residential rehabilitation.
• Assist individuals to access appropriate alcohol and other drug, health, wellbeing and community support services.
• Support re-engagement for people who may be at risk of disengaging from treatment or recovery supports.
• Work collaboratively with the wider Tokoko Integrated Recovery Pathways team to strengthen coordinated care and recovery outcomes.
• Promote recovery-oriented, strengths-based and trauma-informed practice.
• Contribute to service development, client engagement initiatives and continuous improvement activities.
• Maintain accurate documentation, reporting and client records.
