Job summary

Lives Lived Well in Bundaberg is seeking an experienced Team Leader - Programs to establish and lead the Tokoko Integrated Recovery Pathways Program in the Wide Bay region.

You will oversee a multidisciplinary team delivering care coordination, counselling, peer support, cultural support, transition support and post-treatment recovery services, guiding the program from inception to sustainable delivery.

Qualifications

• Leadership experience within AOD, mental health or related human services.
• Experience supervising multidisciplinary teams.
• Strong recovery-oriented, trauma-informed and person-centred practice.
• Excellent stakeholder engagement and partnership-building.
• Experience supporting people with complex needs and managing clinical risk.
• Excellent communication, facilitation and relationship management.
• Current Australian Driver Licence.

Responsibilities

• Provide strategic, operational and clinical leadership to the Tokoko program.
• Lead a multidisciplinary workforce and support professional development.
• Promote evidence-based, recovery-oriented and trauma-informed practice.
• Support risk management, care planning and continuity of care.
• Oversee program implementation and ongoing development.
• Build and maintain relationships with health, housing, community and government stakeholders.
• Contribute to program evaluation, reporting and sustainability.

We are seeking an experienced and passionate Team Leader - Programs to establish and lead our newly funded Tokoko Integrated Recovery Pathways Program in the Wide Bay region.

This is an exciting opportunity to help shape an innovative service model designed to strengthen support before, during and after residential rehabilitation. As Team Leader you will provide leadership to a multidisciplinary team delivering care coordination, counselling, peer support, cultural support, transition support and post-treatment recovery services, helping people build sustainable pathways to recovery and wellbeing.

About the Tokoko Integrated Recovery Pathways Program

The Tokoko Integrated Recovery Pathways Program is a newly funded initiative designed to strengthen recovery outcomes for people accessing residential alcohol and other drug treatment in the Wide Bay region.

Working across Bundaberg and Hervey Bay, the program enhances the existing residential rehabilitation pathway through:

• Pre-admission engagement and treatment readiness support
• Alcohol and Other Drug (AOD) care coordination and navigation
• Transitional accommodation and recovery pathways
• Post-residential recovery and relapse prevention support
• Harm minimisation and psycho education.
• Peer and lived experience support
• Family and community engagement
• Enhanced referral pathways and service integration
• Digital recovery support through ConnectWell

The program aims to improve continuity of care, strengthen recovery outcomes and help people remain connected to support before, during and after treatment.

About the Role

As the Team Leader - Programs, you will provide strategic, operational and clinical leadership across the Tokoko Integrated Recovery Pathways Program.

You will work closely with internal teams, community partners and stakeholders to support the establishment and delivery of integrated recovery-focused services throughout Bundaberg, Hervey Bay and the wider Wide Bay region.

Key responsibilities include:

• Providing leadership, supervision and professional support to a multidisciplinary workforce.
• Promoting evidence-based, recovery-oriented and trauma-informed practice.
• Supporting effective risk management, care planning and continuity of care.
• Leading implementation and ongoing development of the program.
• Supporting workforce capability, quality improvement and service excellence.
• Building and maintaining strong relationships with health, housing, community and government stakeholders.
• Supporting culturally safe and inclusive service delivery.
• Contributing to program evaluation, reporting and sustainability activities.

This role is based in Bundaberg and will involve regular travel throughout the Wide Bay region.

What We’re Looking For

You will be a confident and collaborative leader who is passionate about supporting people affected by alcohol and other drug challenges.

To succeed in this role, you will bring:

• Tertiary qualifications in Social Work, Psychology, Counselling, Nursing, Occupational Therapy, Human Services or a related discipline , and/or relevant experience.
• Demonstrated leadership experience within Alcohol and Other Drug (AOD), Mental Health or related human services settings.
• Experience providing supervision, mentoring or leadership to multidisciplinary teams.
• Strong understanding of recovery-oriented, trauma-informed and person-centred practice.
• Experience supporting people with complex needs and managing clinical risk.
• Highly developed stakeholder engagement and partnership-building skills.
• Excellent communication, facilitation and relationship management abilities.
• Demonstrated commitment to culturally safe and inclusive practice.
• Current Australian Driver Licence.

Experience implementing new programs, services or funded initiatives will be highly regarded.
