Job summary

Lives Lived Well in Queensland is seeking a Clinician to join the Tokoko Integrated Recovery Pathways Program. You will strengthen recovery outcomes for people accessing alcohol and other drug treatment across the Wide Bay region via ConnectWell, a digital wellbeing platform.

You will deliver flexible, recovery-focused support before admission, during treatment and after discharge, and help individuals re-engage with services when needed. Hybrid or remote opportunities apply.

Qualifications

• Qualifications in Social Work, Psychology, Counselling, Occupational Therapy, Nursing, Human Services or related discipline, and/or relevant experience.
• Experience in alcohol and other drug, mental health or community services sectors.
• Experience supporting engagement, recovery, care coordination or continuity-of-care initiatives.

Responsibilities

• Deliver recovery-focused support and engagement through ConnectWell's digital platform.
• Conduct regular recovery check-ins with individuals preparing for or transitioning from residential rehabilitation.
• Support ongoing client engagement and continuity of care throughout treatment and recovery.
• Provide follow-up support after discharge from residential rehabilitation.
• Assist individuals to access appropriate alcohol and other drug, health, wellbeing and community services.
