Job summary

Aligned Community Care is seeking a Clinical Engagement and Development Coordinator to build relationships with participants, families, support coordinators and referral partners across Sydney and Wollongong. The role focuses on intake, referrals and connecting participants with appropriate supports.

You will work closely with the head of operations and growth team, manage end-to-end intake, and ensure high-quality documentation, confidentiality and best-practice across all engagements.

Qualifications

• Tertiary qualification in Occupational Therapy, Social Work, Nursing, Allied health.

Responsibilities

• Manage the end-to-end intake pipeline - initial enquiry and intake assessments, needs assessment, participant compatibility assessment and onboarding.
• Explain available services, support options and funding pathways to prospective participants and their families.
• Build and maintain referral relationships with Allied Health Specialists - Support Coordinators, OT, Plan Managers, LACs, BSP, Public Guardians etc.
• Complete comprehensive, audit-ready intake documentation and maintain accurate client records in line with NDIS Quality and Safeguards Commission requirements.
• Coordinate with internal teams to match participants with appropriate supports.
• Take the lead role in managing external stakeholder correspondence on behalf of the organisation.
• Track referral sources and outcomes and report on pipeline performance.
• Coordinate with various departments across Aligned Community Care to ensure effective collaboration and alignment of organisational activities with business needs.
• Liaise with external agencies and government bodies, and maintain confidentiality in line with privacy and data protection requirements
• Contribute to continuous improvement of intake, onboarding and stakeholder communication processes.

$95,000-$105,000 base salary + Performance-based incentive scheme separate to your salary (subject to experience & qualifications)

This is a full-time, permanent position, based at our Austral office with travel across Sydney and Wollongong. This is not an entry-level role, we're looking for someone with demonstrated experience in the sector. You'll play a key role in building relationships with participants, families, support coordinators, allied health professionals and referral partners.

You’ll manage new referrals, strengthen referral pathways and ensure every enquiry is handled promptly and professionally. Working closely with our head of operations and growth team, you’ll help connect participants with the right supports while building strong, lasting partnerships across the community.

Your responsibilities

What we're looking for

An established professional network within the NDIS sector - Support Coordinators, Plan Managers, LACs, allied health providers, hospital discharge planners and justice liaison contacts

Proven experience in NDIS intake, referral management, support coordination, case management or business development within a disability, health or community services setting, with credibility in front of clinical professionals

Solid working knowledge of the NDIS: plan structures, funded support categories, pricing arrangements and price limits, SIL and SDA, the NDIA planning and review process, and how participants come into the scheme and what happens at discharge

Familiarity with the NDIS Practice Standards, Code of Conduct and Quality and Safeguards Commission expectations, including incident and restrictive practice reporting

Experience supporting participants with complex needs - psychosocial disability, forensic or justice involvement, acquired brain injury, or high-intensity supports - is highly regarded

A background in allied health (psychology, occupational therapy, Behaviour Support Practitioner or similar), social work, or support coordination, is advantageous

A recognised mental health or clinical qualification, current study toward one, or equivalent relevant work experience, is a bonus

Prior experience liaising between clinical, government and community stakeholders on participants' behalf is well regarded
