Job summary

Workforce Extensions FNQ Health is seeking a Registered Nurse to join as Clinical Care Coordinator in Cairns. The role is full-time, community-based with time in participant homes and travel across Queensland, supported by an office in Cairns.

You will manage complex caseloads, develop and review care plans, and collaborate with families, guardians, and health professionals to ensure safe, high-quality supports in line with NDIS standards.

Qualifications

• RN with current unrestricted AHPRA registration.
• 2–3 years post-registration experience in community, disability, or complex care.
• Strong communication and relationship-building with participants, families and MDTs.

Responsibilities

• Manage caseloads of participants with complex care needs and current care plans.
• Develop, implement and review individualised care plans and risk assessments.
• Conduct risk reviews and escalate concerns via clinical governance.
• Liaise with participants, families, guardians, allied health professionals and external providers.
• Manage clinical incidents and documentation per NDIS Practice Standards.
• Deliver clinical training to support staff in line with NDIS High Intensity Skill descriptors.
• Provide wound and catheter care in the community.
• Produce NDIS reports such as continence and wound reports.
• Act as a clinical escalation point for support workers.
• Monitor compliance with NDIS Practice standards and contribute to clinical improvements.

The Opportunity

We are looking for a passionate and experienced Registered Nurse to join our team as a Clinical Care Coordinator. This is a new role supporting our Director/Registered Nurse overseeing the planning and delivery of client centred supports with a focus on quality and compliance.

This is a Full Time role based in our Cairns office however the role is community based so you will spend a lot of time out in our participant homes. You will work across a caseload of participants with complex and high-acuity support needs, working closely with participants, their families, allied health professionals, support coordinators, and our internal teams to ensure the highest standard of support is delivered consistently and safely.

This role will have some travel requirement (Within QLD) whereby accommodation and vehicle will be provided.

What You Will Be Doing

• Managing a caseload of participants with complex care needs, ensuring care plans are current, person-centred, and reviewed on schedule
• Developing, implementing, and reviewing individualised care plans and risk assessments.
• Conducting participant risk reviews and escalating concerns through appropriate clinical governance frameworks
• Liaising with participants, families, guardians, allied health professionals, and external providers
• Managing clinical incidents and documentation in line with NDIS Practice Standards
• Delivery clinical training to support staff specific to participants in line with the NDIS High Intensity Skill descriptors.
• Wound Care in the community
• Catheter Care including SPC and IDC routine changes/flushes.
• Performing NDIS specific reports such as continence assessments & wound reports.
• Acting as a clinical escalation point for support workers
• Contributing to the continuous improvement of clinical practice and participant outcomes across the organisation
• Monitoring and ensuring compliance with the NDIS practise standards

Role Requirements

• Registered Nurse (Division 1) with current unrestricted AHPRA registration
• Minimum 2-3 years post-registration experience, ideally with community, disability, or complex care exposure
• Strong communication and relationship-building skills across participants, families, and multidisciplinary teams
• Confidence managing clinical risk, incidents, and documentation
• Experience in the NDIS or community field is highly valued
• A proactive, self-directed work style with the ability to manage competing priorities effectively
• Experience with clinical management systems
• Current driver's licence
• Yellow and Blue Card
• Experience providing training to support staff/Care workers
• A can do attitude with a focus on exceeding our participant expectations.
