Job summary

Anglicare Southern Queensland in Townsville seeks a Client Liaison to build trusting relationships with clients and coordinate home care and disability services. You will undertake inductions, assess needs, and support goal achievement for consumer-directed care.

The role spans full-time, part-time and casual arrangements with a strong emphasis on collaborative care, budget awareness and compassionate communication.

Qualifications

• Proven ability to manage client relationships and advocate for clients.
• Experience with life-journey planning, goal setting and service review.
• Knowledge of budget monitoring within set parameters.

Responsibilities

• Provide first point of significant client contact to build trust and confidence.
• Conduct home visits to assess needs and preferences and support goals.
• Implement and monitor client agreements to align with program guidelines.
• Build collaborative relationships with clients, carers and advocates.
• Serve as primary contact for service delivery matters.
• Maintain a client-centred approach in all engagements.
