Job summary

Anglicare Southern Queensland in Robina is seeking a full-time Client Liaison to support home care and disability services. You will engage in client relationship management and packaged care case management with clients, their carers, family and advocates, ensuring clear communication and respectful service delivery.

The role involves assessment, planning, and budgeting within consumer directed care, plus coordinating with the Concierge, Multidisciplinary Team Coordinator and Service

Qualifications

• Valid Aged Care Worker Screening Check and current National Police Check or NDIS Worker Screening Clearance are required.
• Current Drivers Licence and own reliable vehicle.
• AQF Level 4 qualification in ageing support or disability and knowledge of case management.
• Experience in community services or similar organisation and working with funded packages.

Responsibilities

• Engage in client relationship management and packaged care case management with clients, their carer and family.
• Coordinate assessment, support planning, and budget monitoring to ensure care packages are sustainable.
• Collaborate with Concierge, Multidisciplinary Team Coordinator and Service Coordinator to deliver CDC services.
• Monitor client budgets and ensure services align with program guidelines.
