Anglicare Southern Queensland in Robina is seeking a full-time Client Liaison to support home care and disability services. You will engage in client relationship management and packaged care case management with clients, their carers, family and advocates, ensuring clear communication and respectful service delivery.

The role involves assessment, planning, and budgeting within consumer directed care, plus coordinating with the Concierge, Multidisciplinary Team Coordinator and Service

Qualifications

• Valid Aged Care Worker Screening Check and current National Police Check or NDIS Worker Screening Clearance are required.
• Current Drivers Licence and own reliable vehicle.
• AQF Level 4 qualification in ageing support or disability and knowledge of case management.
• Experience in community services or similar organisation and working with funded packages.

Responsibilities

The pay-rate for the role will range from $42.15-$44.24/hr depending on your experience.

About the role:

We are seeking a full-time permanent passionate and skilled Client Liaison to join our Robina service and make a meaningful impact in the lives of our clients. Your role will be to engage in client relationship management and packaged care case management with clients, their carer/s, family and advocates for home care and disability
services. The key requirement is effective working relationships with key personnel including
Concierge, Multidisciplinary Team Coordinator and Service Coordinator.

The role may undertake assessment, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client to achieve their identified goals. In addition, this role supports the client to manage and monitor their budget as appropriate to ensure care packages are financially sustainable, and monitors the services provided to ensure these are in accordance with relevant service models and guidelines.

The Client Liaison role ensures that the clients Packaged Care Agreement is met consistent with Anglicare's commitment to Consumer Directed Care (CDC). This approach ensures the client:

• retains ownership of decision making throughout the support planning process;
• determines the level of their involvement in the management of their packages;
• exercises choice in the way services are offered and delivered;
• receives an individualised budget that demonstrates what funding is available under the package and monthly statement detailing income and expenditure against the package;
• is active in ongoing monitoring and review including support to re-assess needs to ensure the package continues to respond effectively to their needs and preferences; and
• May choose to 'top up' their package by purchasing additional care and services.

About you:

• Possess clarity in attitudes based on respectful relationships which reflect a commitment to instilling hope and sustaining capabilities of the client through solution focussed practice.
• Demonstrated proficiency in Client Relationship Management (the ability to build trust, maintain rapport, form collaborative relationships with clients, carers, significant others)
• Demonstrated case management skills (assessment, support planning, review, advocacy)
• Experience with a community services, or similar organisation and ability to demonstrate a sound understanding of the needs and experiences of people who receive community care services and of the community care system.
• Ability to communicate verbally and in writing to a range of stakeholders in a clear and structured way.
• Ability to apply knowledge of organisation processes and resources to assist clients.
• Experience in or ability to acquire skills in monitoring and/or analysis of expenditure to budget and working within set budget parameters.
• Demonstrated negotiation and conflict resolution skills including the ability to negotiate between
  individuals, communities and organisations.
• Knowledge and experience in the Microsoft Office suite and exposure and/or ability to acquire skills in a consumer database.

Role: Requirements:

• Valid Aged Care Worker Screening Check:
• Current National Police Check OR
• Current NDIS Worker Screening Clearance.
  NB: NDIS Worker Screening Clearance is mandatory for Services/Facilities delivering
  services to NDIS clients.
• Statutory Declaration if you have been a citizen or permanent resident of a country
  other than Australia after turning 16.
• Current Drivers Licence and own reliable vehicle
• Dependent on Service based staffing models, this role is required to possess:
• Minimum AQF Level 4 qualification in ageing support, disability or equivalent and;
• Attainment of nationally recognised competency in case management or equivalent or
  willingness to attain
