Job summary

Anglicare Southern Queensland seeks a Client Liaison to engage in client relationship management and packaged care case management for home care and disability services. You will build trust, conduct home visits, assess needs and goals, and support planning to empower clients and carers.

You will maintain client-centered engagement, coordinate with families, and ensure services align with program guidelines while upholding compassionate care standards.

Qualifications

• Clarity in respectful client relationships to support hope and capability.
• Proven ability to build rapport with clients, carers and advocates.
• Experience in assessment, planning and advocacy within community care.

Responsibilities

• Be the first significant contact to build trust and confidence in Anglicare.
• Undertake home visits to assess needs, set goals and monitor client capacity.
• Keep client agreements current and aligned with Anglicare guidelines.
• Build collaborative relationships with clients, families and carers.
• Act as the primary contact for service delivery.
