Job summary

Anglicare Southern Queensland is seeking a dedicated Client Liaison to engage in client relationship management and packaged care case management for home care and disability services. You will be the primary contact, perform home visits, gather needs data, and ensure plans align with Anglicare guidelines.

The role requires a Certificate IV in ageing support or disability and a nationally recognised case management credential, with strong communication and budgeting skills.

Qualifications

• CRM experience building trust and rapport with clients and carers.
• Case management skills: assessment, support planning, review, advocacy.
• Experience in community services and understanding of the community care system.
• Clear written and verbal communication with varied stakeholders.
• Ability to monitor expenditure within budget parameters.
• Familiarity with Microsoft Office and consumer databases.

Responsibilities

• Provide first point of significant contact to build client trust.
• Undertake home visits to assess needs and preferences and set goals.
• Implement client agreements consistent with program guidelines.
• Build collaborative relationships with clients, family and carers.
• Act as the primary contact for service delivery matters.
• Maintain a client-centered approach in all engagements.
