Qualifications

• Minimum two years’ experience in Aged Care, Disability or Community Care.
• Experience developing care plans and coordinating supports to meet client needs.
• Current Queensland driver’s licence.
• Strong communication and relationship-building skills.
• Excellent organisational skills and attention to detail, particularly with documentation and budgets.
• Sound judgement, problem-solving skills and ability to manage competing priorities.
• Commitment to person-centred practice, cultural awareness and positive client outcomes.

Responsibilities

• Build strong relationships with clients, families and support networks.
• Conduct client intakes, assessments and reviews to identify individual needs and goals.
• Develop, implement and review personalised care plans.
• Coordinate services and work with internal teams and external providers.
• Monitor client wellbeing and adjust supports as needs change.
• Manage client budgets and ensure services align with funding requirements.
• Promote client choice, independence and dignity in care.
• Maintain accurate client records and compliance.
• Identify opportunities to improve service delivery and client engagement.
