Job summary

STAR Community Services is seeking an organised Client Intake and Assessment Coordinator in Cleveland to lead intake, assessments and reviews across our community programs. You will supervise intake staff, coordinate referrals and ensure smooth service transitions for clients.

You will apply your experience with My Aged Care, CHSP and Support at Home, manage client records and streamline assessment processes. A driver licence and willingness to travel to STAR locations are essential.

Qualifications

Experience in client intake, service needs assessment, onboarding or review coordination.
Experience supervising or coordinating staff or volunteers.
Working knowledge of My Aged Care, CHSP and Support at Home.
Strong documentation skills and experience using client management systems.

Responsibilities

Supervising intake staff and volunteers.
Coordinating enquiries, eligibility screening, onboarding and referrals.
Undertaking in-home service needs assessments for clients and handover to Care Partners.
Managing My Aged Care portal workflows and client records.
Identifying risks and coordinating escalation and follow-up.
Monitoring assessment documentation, care plans and reviews.
Collaborating with service delivery teams to improve processes.
