Job summary
STAR Community Services in Cleveland, QLD is seeking an experienced CHSP Program Team Leader to provide operational leadership across our CHSP services. You will lead Coordinators, ensure service delivery aligns with Strengthened Aged Care Quality Standards, and support the transition to the new Support at Home Program.
The role focuses on building high-performing teams, driving continuous improvement, managing budgets, and fostering partnerships with healthcare professionals and community
Qualifications
- Tertiary qualification in Community Services, Aged Care, Social Work or related field.
- Experience leading multidisciplinary teams in aged care or community services.
- Strong knowledge of CHSP service delivery, compliance and aged care reforms.
- Experience managing budgets, reporting and quality improvement activities.
Responsibilities
- Lead and mentor CHSP Coordinators to deliver high-quality services.
- Oversee CHSP service streams and ensure person-centred, safe outcomes.
- Monitor budgets, performance, and contract obligations.
- Ensure CHSP compliance with funding guidelines and quality standards.
- Build relationships with My Aged Care, health professionals and stakeholders.
