Job summary

Life Without Barriers in Mackay is seeking a Child and Family Practitioner to provide high-quality case management and support to children and young people living in family-based care. Based in our Mackay office, this is a permanent full-time opportunity offering meaningful work, flexibility and ongoing professional development.

You will collaborate with foster carers, Child Safety, families and community stakeholders to develop and implement individualized care plans and strengthen placement

Qualifications

• A recognised degree in Social Work, Psychology or a related discipline.
• Experience in child protection and/or out-of-home care.
• Trauma-informed and culturally responsive approach.
• Experience supporting foster carers and managing complex casework.

Responsibilities

• Deliver child-focused case management, planning and assessments.
• Build partnerships with foster carers, Child Safety and families.
• Provide ongoing support and supervision to Foster Carers through home visits.
• Support carers to implement case plans and achieve positive outcomes.
• Develop and monitor risk, cultural, safety and behaviour support plans.
• Maintain accurate case notes and foster care records.
• Contribute to a collaborative, child-focused team environment.
