Job summary

Life Without Barriers is seeking a dedicated Child and Family Practitioner for our Mackay office. This permanent full-time role focuses on high-quality case management and delivering supportive services to children and young people in family-based care arrangements.

You will work with foster carers, Child Safety and families to develop individualized care plans, strengthen placement stability and support positive outcomes, with opportunities for professional development in a respected

Qualifications

• Recognised degree in Social Work, Psychology or related discipline.
• Experience in child protection and/or out-of-home care.
• Trauma-informed and culturally responsive approach.
• Experience supporting foster carers and managing complex casework.
• Excellent communication, organisational and relationship-building skills.
• Current Australian Driver Licence.

Responsibilities

• Deliver child-focused case management, planning and assessments.
• Build partnerships with foster carers, Child Safety, families and stakeholders.
• Provide ongoing supervision and guidance to Foster Carers via home visits and contact.
• Support carers to implement case plans and achieve positive outcomes for children.
• Develop and monitor risk assessments, cultural and safety plans.
• Maintain accurate case notes, foster care agreements and renewals.
• Contribute to a collaborative, child-focused team environment.
